If you live in Buchanan County, have a disability and have experienced homelessness for an extended period of time or on multiple occasions over several years, we can help connect you with stable housing. Our Continuum of Care model means we will provide you support along your journey to stability.
Disabilities include physical health conditions, mental health challenges, and substance use disorders.
This program is only available to residents of Buchanan County.
