Catholic Charities of Kansas City-St. Joseph Senior Care Services (SCS) provides a menu of supportive services for older adults (55+, low-income) in neighborhoods where underserved seniors reside.
The program provides a comprehensive, client-centered continuum, designed to meet a senior’s varying needs. This allows seniors to live a healthy, self-sustaining life in their own home and community.
In-Home Services include:
- Meal preparation for special diets
- Cleaning
- Laundry & ironing
- Trash disposal
- Shopping, essential errands
- Reading
- Resource education
How to get started?
Home-based service participation begins with an initial conversation either with the individual and/or the referral source. A registered nurse visits the client’s home, assesses needs, and reviews individualized care plans. Once the in-home assessment is complete, services are scheduled.
Services will be provided by a skilled Certified Nurse Assistant (CNA) daily, weekly, or monthly, depending on the individual’s needs. Schedules are designed for the same caregiver to be provided, building rapport, ensuring greater continuity of services, but most importantly, instilling a sense of self-dignity.
Care Management:
A key component to Senior Care Services is to provide the knowledge and skills necessary to access resources within the community, social and governmental systems specifically tailored to seniors. Through experience and ongoing interaction, our CNA can identify the potential for additional services needed, such as:
- Nutrition education
- Access to senior benefits
- Deeper social interaction and integration
Of course, if an individual’s needs go beyond the scope of staff, referrals will be provided, and accommodations made for access to those services.
Community Based Wellness Screenings:
Older adults often suffer from the lack of regular preventative wellness screenings including checking blood pressure, heart rate, respiratory, or blood sugar levels. Catholic Charities of Kansas City-St. Joseph offers regular wellness screenings in community locations that include low-income housing properties, parishes, food pantries, community centers, community meal locations, and social service agencies. This can head off problems before they occur by providing recommendations and referrals based on results.
